Drug cost category for many managed care organizations MCOs ; . Priloseec OTC was approved on June 20, 2003, as the first PPI for over-the-counter use. Although Rpilosec OTC is only indicated as a 14-day therapy regimen for treatment of frequent heart b u rn, it is speculated that prescription PPI utilization will decrease as more health plan members use the over-the-counter product. METHODS: The health plan, as part of a fully integrated health system, has instituted measures in order to decrease drug spend on the PPI class. Several thousand coupons for 0rilosec OTC were sent to occasional PPI-utilizing members along with a letter describing appropriate use of the OTC medication. The health plan also emphasized the Prilosc OTC initiative during employer group meetings and physician detailing visits. The health system's electronic medical record system was used to identify member utilization for all PPI use during the last 3 years. This data was evaluated for trends in utilization, cost to members, and cost avoidance to members and the health plan. RESULTS: The analyzed data showed a cost avoidance of more than 0, 000 for the 1-year period following implementation of the initiatives. The data also showed a trend of decreased per-memberper-month PMPM ; costs. Priloaec OTC had distributing shortages throughout the year. The health plan may have seen an even greater shift toward cost avoidance had the distribution shortages not occurred. CONCLUSIONS: Implementing a program to increase use of Prilosec OTC through coupon mailers, physician education, and employee group instruction showed significant cost avoidance for the health plan. Despite an increase of ingredient cost per PPI prescription, the PMPM cost decreased. Other ideas, such as coverage of OTC products, needs to be evaluated.
